FACTORS FOR FAILURE OF TYMPANOPLASTY FOR TOTAL AND SUBTOTAL PERFORATIONS

OBJECTIVES: Results of Tympanoplasty for Total or subtotal perforations of the tympanic membrane perforation are often not satisfactory. Various causes have been identified for failures. Hereby we present our study about various factors for failures of tympanoplasty for total/subtotal perforations.

METHODS: In this randomized retro-prospective study, 88 cases who under went revision tympanoplasty surgery for total/subtotal perforation from June’1996 to till date were included. Only patients, who had undergone tympanoplasty for total/subtotal perforation earlier, were included in this study. All patients underwent detailed evaluation. Various factors for failure were classified into five groups such as Anatomical factors, Pathological factors, Surgeon factors, Patient factors and technical factors.

RESULTS: Each and every factor mentioned above has important bearing on the outcome of tympanoplasty for total/subtotal perforations and failure is often due to multiple factors. Pre-operative pathology, experience of the surgeon and the surgical technique used, has important implication on the results of tympanoplasty.

CONCLUSION: Pre-operative identification of potential risk factors for failure of tympanoplsty for total/subtotal perforation is important. Understanding these factors can help in the improvement of results. This study attempts to identify such potential risk factors.
